If you are interested in making an appointment to visit our nurse for health related services. Please contact our office at 972-953-7669 to book an appointment. You can also click the NURSE button on the right to send an email requesting an appointment and we will be in touch with you. If you are new to our office, please print and complete the INTAKE FORM on the right. We offer free blood pressure, $3 blood sugar checks and $20 appointments for fingernail or toenail clipping or $25 appointments for ear cleaning. Low cost blood draws/testing are also available.
Counseling Services
If you are interested in making an appointment to visit our mental health counselor, please contact our office at 972-953-7669 to book an appointment. You can also click the COUNSELOR button on the right to send an email requesting an appointment and we will be in touch with you. Please Identify if you are in a CAREGIVING role. If you are new to our office, please print and complete the INTAKE FORM on the right.
